This is the registration form for Two Years Old through 5th Grade. Sunday School starts September 13 during the Education Hour!
Sunday School will be held from 10:10 to 10:50.
Parent/Gardian Information
Sunday School will be held on the lower level.
Little Lambs - PreK (ages 2-4) will start in their classroom (room 102).
K-5th will start in room 150 and then go to their classrooms.
Pick up will be in each child's classroom.
If your child should be released to another adult than the one who signed them in OR released on their own, please indicate preferred arrangment below.
The GSLC Sunday School program is volunteer-led.
As part of the registration process and our 'partnership in ministry program', each family is asked to help out in the classroom at least once per semester. There is no guarantee that you will be assisting in your child's classroom.

I understand images of my child may be used throughout Good Shepherd, including, but not limited to: the website, internal postings, the directory, the newsletter and other related putlications. By participating in this program, I grant the irrevocable right to use such photos in all forms and media and in all manners, for said purposes. I hereby forever waive any right to inspect or approve the finished product, including but not limited to, written copy and/or an image in print or on a website that may be created in connecdtion therewith. I understand that Good shepherd cannot control the unauthorized use by personos other than Good Shepherd of said image once such image is putlished. Any claim I may have concerning unauthorized publication of said image must be pursued by me against the unauthorized user. Good Shepherd disclaims any responsibilty for such unauthorized use of the published image.
